The Form Submission report displays data on form submissions, such as registrations for a specific webinar. You can select specific forms to view the form fields and how people filled them out. The report also includes any custom and hidden fields on forms.  

To download a Form Submission report: 

1. Go to Reporting > Report Center.
2. Choose the Form Submission template and click Open Template. The Form Submission report filters open in the right panel.
   
    
3. Use the following filters to focus on specific data in the Form Submission report: 

• Form: select either the 'Is' or 'Is Not' filter option, depending on whether you want to include or exclude specific values. Then, choose the specific form(s) from the list which you want to include or exclude. Or, search the list by form name or form ID. The form ID is presented on the form in the Designer.
  
  
   
• ​​​​​​​Filter by:
  • Boards: the default filter
  • Channels

After selecting the boards or channels filter, select either the 'Is' or 'Is Not' filter option, depending on whether you want to include or exclude specific values. Then, choose the specific boards and channels from the list which you want to include or exclude. Or, search the list by name or ID. The default is All, meaning the report will display all boards, or all channels. 
To show your selected choices, click SHOW SELECTED.

• Date Range: set the desired date range using the calendar picker. The default is for the last 3 months.
• Lead Type: select a lead type from the drop-down list; All, External, or Internal. The default filter is external.
• Accounts: select either the 'Is' or 'Is Not' filter option, depending on whether you want to include or exclude specific values. Then, choose the specific account(s) from the list which you want to include or exclude. Or, search the list by account name or domain. 

4. After filtering the report, click Download Report.

The Form Submission report is exported in a CSV file and includes the following data:

Lead Domain	Domain of the lead's account
Board Name	Name of the board
Board ID	ID of the board
Form ID	ID of the form (can be found in the designer)
Form Type	Type of form (Classic, Marketo, Eloqua, Custom)
Form Name	Name of the form
Cta Action	CTA action (Form, Registration, Gated)
Filled In Form On	Date and time the lead filled in the form
Form Privacy Message	Form's privacy message
Form Privacy Checked 1(Y/N)?	Indicates if the lead agreed and checked the first privacy message, if exists
Form Privacy Checked (Y/N)?	Indicates if the lead agreed and checked the second privacy message, if exists
	Note: The report displays fields that are set in the form and the respective responses provided by the lead for each field.
